OSHA – Meatpacking District fatality ‘completely avoidable’

According to a report released this week by OSHA the death of the 22 year-old worker from Queens, Carlos Moncayo, was ‘completely avoidable’.  Moncayo was buried alive when the 14 foot deep trench he was working in for a new Restoration Hardware collapsed.
